Program Overview

This year-long program is designed to evaluate, monitor, and improve bone density, metabolic bone markers, hormonal balance, inflammation, and musculoskeletal strength. The protocol integrates advanced imaging, laboratory investigation, nutrition, lifestyle, and individualized hormonal/supplement support to optimize skeletal health and prevent osteopenia and osteoporosis progression.

Program Components

1. Advanced Bone Imaging (Gold Standard)

DEXA – 2 Scans per Year
• Scan #1: Baseline DEXA + body composition
• Scan #2: 6–12 months into program to measure response to interventions

Clinical Measurements Assessed
• Lumbar spine, femoral neck & total hip BMD
• Body composition (lean mass, visceral adiposity, sarcopenia scoring)
• Delta-change analysis comparing baseline → follow-up

Clinical Rationale
• Visceral fat strongly correlates with chronic inflammation → increased bone resorption
• Lean mass improvements track treatment success beyond density alone

2. Comprehensive Laboratory Evaluation

A. Baseline Blood Work (LabCorp – insurance-eligible where covered)

Used to diagnose underlying contributors to osteopenia/osteoporosis:
Bone Metabolism Markers
• Calcium, albumin-corrected calcium
• Phosphorus
• PTH (hyperparathyroidism = major reversible cause of bone loss)
• 25-OH Vitamin D
• Bone turnover markers: CTX & P1NP (baseline)

Thyroid & Metabolic Drivers
• TSH, Free T4, Free T3
• Hemoglobin A1c, fasting insulin
• Lipid panel (integrated into cardiometabolic risk for bone health)

Electrolytes & Micronutrients
• CMP
• Magnesium (RBC preferred – intracellular status)
• Iron panel + ferritin (if fatigue or hair loss → bone health links)

Inflammatory Markers
• hs-CRP; ESR

Sex Hormones
• Testosterone (total/free); DHEA-S

B. DUTCH Complete Hormone Test

Evaluates hormonal and metabolic patterns highly relevant to bone physiology:
• Estrogen, progesterone, androgen levels
• Cortisol diurnal pattern (cortisol dysregulation → bone resorption)
• Phase I/II estrogen metabolism (2-OH/4-OH/16-OH pathways)
• Organic acids:
  o B-vitamin sufficiency
  o Mitochondrial markers
  o Neurotransmitter precursors
  o Oxidative stress indicators
 
Medical Justification
• Estrogen deficiency = #1 cause of rapid postmenopausal bone loss
• Dysregulated estrogen detoxification → inflammatory burden → worsened bone turnover
• Cortisol excess reduces osteoblast activity

C. GI-MAP Stool Test

• Assesses mineral absorption; inflammation; dysbiosis
• Gut-derived bone loss contributors (LPS, steatocrit, pancreatic elastase)

Appointment Structure (4 Visits / Year)

Every other month 10-Minute Check-Ins between full visits help maintain momentum, accountability, and safety.

Clinical Management Included

Hormonal Support (as medically indicated)

• Bioidentical estrogen, progesterone, testosterone
• Support for estrogen detoxification (DIM, sulforaphane, calcium-d-glucarate)
• Monitoring levels & adjusting doses based on symptoms + labs + DUTCH

Medication Review & Optimization

• Evaluate meds contributing to bone loss (PPIs, SSRIs, steroids, etc.)
• Recommendations for alternatives where appropriate
• Support for sleep and stress (critical for bone turnover)

Nutraceutical & Supplementation Plan:

• Customized to labs + DEXA findings

Nutrition & Lifestyle Support

• Anti-inflammatory, Mediterranean-style bone-supportive diet
• Protein intake optimization
• Gut health optimization for mineral absorption- optional add on GI MAP
• Stress-resilience practices (vagal tone, adaptogens if needed)

Activity & Physical Training Plans

Tailored to each patient:
• Weight-bearing activity schedule
• Resistance training plan
• High-intensity intervals (when suitable)
• Fall-prevention strategies

Program Outcome Statement

At the end of 12 months, you will know exactly why you were losing bone, what to do about it, how to stabilize your hormonal and metabolic patterns, and you’ll have measurable improvements in your DEXA and strength.

Program Investment & Pricing: Bone Health Optimization Protocol – 12-Month Program

Total Program Fee (One time Pay): $4,500 or pre-arranged Installment Payments

This covers the entire year-long protocol and includes:

  • Our programs are highly customized to your needs, and prices may vary based on recommended testing.
  • All Programs start with an initial discovery session (90 minutes), and the cost is credited towards your personalized blueprint.

Program Investment & Payment Options

Option 1 — Pay in Full

• One-time payment of $4,500

Option 2 — Installment Plan

• $1,500 initial payment
• Eleven monthly payments of $275 

Billing & Reimbursement

  • HSA/FSA funds may be used if available
  • Superbills are provided up to 4 times per year upon request
  • Medicare beneficiaries are not eligible for superbill reimbursement.

Not included in the program fee:

• LabCorp blood work (billed to insurance where applicable)
• BHRT prescriptions (pharmacy cost)
• Nutritional supplements (Fullscript price)
